Inability to conceive children ???

  • The inability of couple to achieve conception after 1 year of unprotected coitus is known as infertility ( primary infertility ) 
  • Is an absolute state of inability to conceive is called as sterility
  • The inability to conceive after an initial phase of fertility is called as  secondary infertility 
  • Infertility can affect either the male or female partner of a marriage
  • 10-15 % of marriages prove to be childless
  • Fertility is a relative rather than an absolute state and comparatively few individuals are fully sterile or fully fertile .Majority fall somewhere in between these two extremes
  • The fertility of a marriage is a sum of the fertilities of the two partners .Low fertility in both partners may result in infertility
  • This explains why some couples fail to reproduce yet ,when they separate and each takes a newmate ,they both proceed to have children
  • Women high fertile age is between 17-34 years .After 34 yr fertility rate gradually falls
  • Men fertility rate gradually falls after 40 yrs

 

WHY INFERTILITY ? 
  • Women are responsible for 40% of infertility
  • Men are responsible for 35% of infertility
  • Combination factors occupies 10-20%
  • Unexplained reasons are 5-15%
FEMALE CAUSES OF INFERTILITY : 
  • Ovary,fellopian tubes, uterus,cervix, vaginal diseases interfere with fertility
OVARIAN FUNCTION:
  • Anovulation causes infertility . Anovulation seen in irregular cycles and with scanty menses .Any alteration in structure or function of ovary leads to irregular cycles and may effect egg release
  • Below are the conditions where, ovarian function effects :
  • PCOS -poly cystic ovarian syndrome
  • Luteal phase deficiency , luteinized unruptured follicle ( in which decrease production of hormones by corpus luteum ,decrease FSH &LH )
  • Premature ovarian failure
  • Resistant ovary syndrome
  • Hyper  prolactinemia ( may be because of drugs,pituitary adenoma , primary hypothyroidism)
  • Diabetes mellitus
  • Sub clinical adrenal failure
  • Sex chromosomal anomalies etc
TUBAL FACTORS :
  • Total or partial occlusion of fellopian tube causes infertility.Occlusion is because of salpingitis ( Infection of tube ) .It may be post abortion ,post delivery or intermittent infections .Commonly gonococcus ,chlamydia and tuberculous organisms effect the tube
UTERINE FACTORS :
  • Uterine problems effect the implantation of embriyo.Following conditions are associated with this
  • Absence of uterus ( congenital anomalies )
  • Atrophy of uterus
  • Hypoplasia of uterus
  • TB endometritis
  • Endometriosis
  • Pelvic inflammatory diseases
  • Intra uterine adhesions ( ashermans syndrome – may be due to excess curettage or previous surgery on uterus )
  • Submucous polyp
  • Uterine leiomyomas
CERVICAL FACTORS : like
  • Problem with cervical mucous – impenetrable / poorly penetrable mucous ,low PH of mucous ,presence of local sperm antibodies
  • Loss of mucous seen in conditions like – amputation of cervix ,cone biopsy , cervical diathermy
  • Faulty direction of cervix seen in retroversion or severe prolapse
  • Cervical stenosis
PERITONEAL FACTORS :
  • These factors interfere with egg transfer to fellopian tube
  • Pelvic adhesions seen in appendicitis,post abortions ,puerperal infections,endometriosis etc
VAGINAL FACTORS :
  • Vaginal tumours , septa and membranes preventing the spermatozoa ascending in to uterus
